weapon missing after dieing, completely insured.

went farming on sampire and got pked. i had my scorpion slayer leafblade in my inventory, but i was using a radiant so it couldnt have broken.

i also didnt get a player murder gump, so i was not attacked by any stealther/player i didnt see.

Has anyone ever had this happen to them?
If a staff could look into the code and find out how this happened, it will be greatly appreciated.

ive checked everywhere Ajani. cleaned out my bank box, checked my insurance menu twice, it's not here.
