Bump:

It has now been 90 days since accounts were refreshed. Any accounts that have not been logged into since then will now have their houses start decay.

House decay takes approximately 5 days, so there is still some time to refresh your account(s) if needed.

The scratch card drops as a Paragon "artifact" and goes directly into your backpack.

With 125 items in your backpack, all insured, and a set of 15 or so insured equipped items, insurance will only cost 84k in gold (600 gold per item). I am not sure how you're reaching 100k per death without something being seriously off.

What does your Item Insurance Menu say your cost per death is?

You may have to select the title after claiming it from the rewards. Click your character and press "Select Reward Title" in the context menu. You can then click "Next" in the gump until you see your desired title, and click "Accept" to apply it.
