If someone is cheating in an exam?

Hi there guys
What would you do if you are at some exam and you see somebody cheating. I mean you've seen them doing it or preparing their chosen method before the exam if you don't say anything could you be penalised for not saying anything?
I mean you should say something obviously but honestly would you really?

Proctoredu and didn't encounter such issues at all.
